Debbie, below is a sample both of how the quiz could be displayed and how the form could be displayed. Note you have the option of having people do "long form" answers, multiple choice or true/false. The form at the bottom could be modified to have whatever fields you want. We could have their answers emailed to you, as well as saved within Neon so they are part of the person's record within the backend (CRM) of the system.
